A proof of delivery, or POD, is the paper the consignee signs and stamps when goods reach the destination. It records the date and time, packages received, any shortage or damage and the receiver's name. Collect it for every trip: it is what lets you raise the freight bill and ask for the balance payment.

| To (destination) | Place where the goods will be delivered. |
| Description of goods | What is loaded, in plain words (for example cement bags, steel coils). |
| Packages dispatched | Number of packages shown on the LR. |
| Packages received | Number the consignee actually counted at delivery. |
| Weight (kg / tonnes) | Actual or charged weight. Write the unit. |
| Shortage / damage | Write nil if all is fine. Otherwise note how many and what. |
| Remarks | Anything special: damage on loading, seal number, delivery instructions. |
| Receiver name and phone | Person who took delivery, so you can call them if needed. |
| Receiver signature and stamp | The consignee signs and stamps on delivery. This is your proof. |
| Driver signature | The driver signs to accept the goods and the document. |
How to fill the POD format

Download the POD format as PDF and print it on A4, or open the Excel or Word file on your phone or computer.

Fill the top fields first: number, date, truck number and the parties. Use a pen on the printed copy or type into the file.

Fill the LR number and packages dispatched before the truck leaves, and hand the sheet to the driver. At delivery the receiver counts, writes the packages received and any shortage, then signs and stamps. Never accept a POD that is signed but has the remarks left unread.

Check the amounts, get the signatures, and give each party its copy. Keep one copy for your records.

Why is a POD important for a transporter?
The signed POD shows the goods reached the consignee in the stated quantity. Customers usually pay the freight bill against it, and truck owners often get the balance only after it comes back. Without it, a shortage claim is hard to answer.
What if the receiver finds a shortage or damage?
Write the exact shortage or damage in the remarks box before the receiver signs, and ask them to sign beside it. Take a clear photo of the signed POD as a backup. Do not let the receiver sign "clean" and complain later.
